Kodak DC215 and it includes usb cord, a 32 MB memory card we bought separately, USB card reader, software and manuals etc.

It does have issues, sometimes it stays and sometimes it doesn't. I think it's mostly because there is a small screw missing from the bottom, if someone could find a screw small enough to fit, I think it'll be fine. We just keep a piece of electrical tape on it.
